Join us as we review a standardized format adopted by medical and other health professionals to detail interaction with clients/patients often referred to as SOAP notes. SOAP notes provide a systematic assessment of a client/patient’s progress and interventions. The session will provide general guidelines on what a SOAP note should and should not include, accommodating various work settings.

A CHWs understanding and utilization of SOAP notes is crucial for several reasons because it is a system used to improve communication among health professionals, others caring for a patient/client but also helps to identify barriers and is often used to bill and/or reimburse an employer and validate insurance claims.
