Patient-Centered Assessment in Transitional Care Management & Coordination Training
The training is designed to equip Community Health Workers (CHWs) and other health professionals with the skills and knowledge needed to conduct effective, patient-centered assessments. By integrating lived experiences and leveraging their roles within team care, participants will enhance patient care and outcomes through improved care coordination. This is the first training course in an eight-part series aimed at supporting CHWs and other allied health professionals as we work to better understand the transition to the Community Health Integration model.
This training is eligible for 2.0 CEs that can be used for certification or renewal purposes.
